The New England Shelter for Homeless Veterans is the nation's largest provider of services to homeless veterans. Through a vast array of services dealing with such diagnosis as PTSD, substance abuse and chronic homelessness, the Shelter, with its educational, training and outreach programs, helps over 2,000 veterans a year obtain the skills necessary to become independent citizens in today's workforce.

Position Title: Case Manager Department: Clinical Services/Case management Full-time Exemption: ( x ) exempt ( ) non-exempt Supervisor: Director, Case Management Department Supervises Position (if applicable): · NA Job Summary: · Work with a team of clinical case managers providing case management and benefits counseling to a homeless veteran population. Responsibilities: · Assess potential clients for the program. · Provide case management services to a caseload of Veterans. · Develop treatment plans and treatment plan reviews with goal of transitioning to permanent or other appropriate housing. · Manage crisis situations with caseload and with other shelter clients as needed. · Provide referrals and follow-up to appropriate collateral services. · Assist clients with increasing or stabilizing their income via public assistance, SSI/SSDI, training/employment, · Maintain appropriate client records and database information for annual reporting. · Perform other duties as assigned by Program Manager. Other skills/experience necessary: · Licensed Mental Health Counselor or Social Worker with a Masters Degree. · Previous experience with chronic mental illness, substance abuse, frail elderly, and/ or homeless population preferred. · Ability to be flexible and comfortable with a multi-cultural, multi-problem population.
Position Title: · Director, Case ManagementDepartment: Clinical Services Exemption: (x) exempt ( ) non-exempt Supervisor: Vice President of Client Services Job Summary: · Manage case management department · Provide clinical and managerial supervision of MSWs and LCSWs · Provide case management and supportive mental health counseling to a homeless population, including post-incarcerated veterans · Serve as a senior manager in the Client Services Department Responsibilities: · Manage a staff of case managers, including filling vacant positions, conducting performance reviews, and providing career development guidance · Assess and enroll potential clients for the post-incarcerated program · Provide supportive counseling and pre-employment assistance to post-incarcerated clients, assisting with re-entry and mental health issues and maintenance of sobriety · Develop treatment plans and treatment plan reviews with goal of transitioning to permanent or other appropriate housing · Manage crisis situations with caseload and with other shelter clients as needed · Provide referrals to appropriate collateral services · Assist clients with increasing or stabilizing their income via public assistance, SSI/SSDI, training/employment. · Maintain appropriate client records and database information for grant reporting · Provide clinical supervision to MSWs and LCSWs preparing for independent licensure · Perform other duties as assigned by VP of Client Services. Other skills/experience necessary: · Must be a Licensed Independent Social Worker (LICSW) · Previous staff management experience required · Previous experience with post-incarcerated populations preferred; experience with chronic mental illness, substance abuse and/or homeless population preferred · Ability to be flexible and comfortable with a multi-cultural, multi-problem population
